TPS2456A: TPS2456A的输出被锁死的问题

meet a issue about TPS2456a’s oring output is locked during system power reboot  processing on our XG480 100G IO card. When output of TPS2456a is locked, 12V is only 0.277V output after ORing circuit and Q3 is not enabled by TPS2456a, but ENA and ENB of TPS2456a are normal. The schematic of TPS2456a as below
